The primary purpose of your job is to provide social work services to residents, patients and their families at admission, during placement and at time of discharge and to provide departmental supervision education and consultation as requested.

Job Description:



  • Coordinates the admission of new residents/patients and assists in the initial adjustment of the resident/patient and family/significant others to include:
  • Reviews the admission packet with the resident/patient and significant others. The packet includes the admission agreement, resident rights, notification procedures, medical consent form and safety procedures
  • Orients the resident/patient and significant others to the facility, addressing any needs or concerns regarding the resident/patient adjustment
  • Provides social work assessments and counseling to assist residents/patients and significant others in their adjustment to the new environment
  • Completes resident/patient assessments and as a member of the ICCP team develops a
  • Interdisciplinary Comprehensive Care Plan for each individual
  • Assesses resident/patient status including strengths, coping mechanisms, personal goals, progress towards goals, significant relationships, and problem areas
  • Develops a plan with the ICCP team that addresses the resident's/patient's problem areas and goals
  • Ensures that resident/patient and significant others are involved in the care plan process
  • Establishes and maintains an on-going relationship with residents/patients and significant others. Assists residents/patients through this relationship to adjust to any medical, social, emotional problems which may arise
  • Continually assesses the resident's/patient's psychosocial strengths and needs and providing counseling, support groups, emotional support and referrals to meet identified needs
  • Acts as a liaison between the family and St. Ann's staff, providing supportive services and interpreting policies and procedures
  • Provides social work input to address psychosocial aspects of all resident/patient care needs such as behavioral issues, psychotropic medication use and physical/cognitive decline
  • Provides resident/patient and significant others with hospice and grief counseling and support
  • Identifies resident/patient discharge potential and educates resident/patient and significant others of discharge options
  • Makes referrals to community agencies to arrange for in home services upon discharge
  • Facilitates referrals to Adult Protective if discharge is against medical advice
  • Informs significant others of residents discharge to another level of care or to an acute care facility
  • Discusses the facility's bed hold policy as appropriate
  • Monitors resident's/patient's emotional status and provides support and reassurance through the discharge process
  • Provides post discharge follow-up to resident/patient and significant others
  • Assists resident/family member with discharge to another facility or lower level of care
  • Provides consultation to BSW social workers on complex cases requiring advance level practice skills in assessment and intervention
  • Provides social work team members with in-service training in advance practice skills
  • Provides facilitation to family and resident groups on an ongoing basis
  • Coordinate student internships for the department.
  • Maintains timely documentation of events and all required paperwork including but not limited to MDS, RAPS, ICCP
  • Coordinates resident/patient group meetings including: Resident Council, and family/resident/patient support and informational groups
  • Coordinates the transportation needs for residents/patients
  • Assists resident/family and significant others in making application to MCDSS for Medicaid and provides interpretation and assistance with health related forms
  • Provides supervision of social work students from area colleges
  • Facilitates the application of Resident's/Patient's Rights in daily care issues.
  • Provide weekend/evening supervisor on call coverage



Candidate Requirements:



  • LMSW preferred
