Program Manager, Population Health Analytics

The Program Manager, Population Health Analytics will oversee a portfolio of analytics programs and partnerships in support of the Yale New Haven Health Clinical Affiliates and the broader enterprise population health strategy. The Program Manager will serve as the primary liaison with internal and external partners, driving data driven decision making to enhance patient outcomes, reduce costs and improve overall healthcare delivery. Reporting to the manager, the Program Manager must be a dynamic individual with a passion for improving health care delivery in the ambulatory setting and across the continuum of care and have a deep understanding of value base care models. The role requires an ability to build robust, collaborative relationships with multiple stakeholders, exemplary project management and organizational skills, strong communication skills, work autonomously with minimal oversight while being a strong team player.

Responsibilities

  • 1. Data Analytics
    • 1.1 Acts as a strategic partner with members of leadership and interdisciplinary teams to produce data analytics solutions that assists in key decision making, population and value-based program management.
    • 1.2 Serves as a mentor and leader within the division and with cross functional partners; develops division competency in advanced population health analytics.
    • 1.3 Defines the overall data and analytic business requirements for the population health team and, in conjunction with Digital Technology Solutions, identifies, develops and implements appropriate technical solutions to support the population health data and analytics strategy
    • 1.4 Works closely with external stakeholders (vendors, payors) and internal partners (DTS, finance) to establish efficient and reliable processes for obtaining and processing various data sources (i.e. claims, clinical and others) required for population health analytics and performance in value based arrangements.
    • 1.5 Develop and implement a robust data validation and quality assurance program for the division to ensure excellence in data stewardship
    • 1.6 Keep current on technology, regulatory and operational trends in the health care and population health industries that may impact utilization.
  • 2. Performance Improvement and Project Management
    • 2.1 Oversees the coordination and execution of a population health analytics strategic plan to include the adoption, optimization and maintenance of data and technology solutions aligned with divisional and organizational goals and priorities.
    • 2.2 Serve as the primary liaison with key partners across the enterprise to ensure population health analytics strategies are coordinated and integrated.
    • 2.3 Establishes standards for program and project management for the analytics function; partners with population health analysts to develop and maintain project plans and key performance indicators for the division and organization.
    • 2.4 Stays informed on best practices in population health analytics, new technologies and capabilities and coordinates evaluations of new solutions for consideration.
    • 2.5 Understand the interrelation of existing population health vendor solutions to identify areas for enhanced operational effectiveness.
  • 3. Network Administration
    • 3.1 Assists with the integrity of provider information and collaborates with internal and external partners to ensure data accuracy; this includes being a key stakeholder in the development and management of our centralized provider directory.
    • 3.2 Routinely perform audits and reviews data to ensure the provider information that is used for administering networks is accurate and complete for the populations the department is responsible for managing.
    • 3.3 Identify opportunities for operational efficiencies in relation to provider management , building upon lessons learned and scaling to future initiatives

Qualifications

EDUCATION

Bachelor's degree required. Master's degree in business, public health, information technology or other related field strongly preferred.

EXPERIENCE

At least 5- 7 years experience working within a health care setting with progressively increasing responsibilities. Advanced skills in data analysis, data visualization, predictive modeling and data manipulation is required. Advanced training or certification in project management, performance improvement and quality improvement methodologies is strongly preferred. MS EXCEL at an advanced level required ; Proficient in SQL and ability to master new skills /programming languages as required. Experience with either claims data or electronic medical record software is required. Comfortable training others on adoption of software tools and other project specific software.

Able to proactively identify opportunities through analytics and drive solutions. Exceptional written and verbal communication skills and comfortable presenting to senior leaders. Strong understanding of relational reporting and associated data analytics techniques and strategies. Ability to work independently and meet deadlines with minimal supervision. Demonstrated ability to work well under tight time constraints and manage time effectively. Strong organizational, problem solving and listening skills, attention to detail, and innovative thinking. Must possess a high level of customer service skills and participate as a strong collaborative team player.

This is a hybrid position, with the office in New Haven, CT.
