This is an entry level clinical pharmacist position. The Home Infusion Clinical Pharmacist I supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills. Qualifications EDUCATION Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree. EXPERIENCE Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desired, but not required. LICENSURE Connecticut Pharmacist Licensure required, active and without restrictions. BLS (Basic Life Support) certified or obtained within 6 months of hire. SPECIAL SKILLS This individual should demonstrate basic knowledge of pharmacology and therapeutics and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. Must have expertise in patient management. Must pass all sterile compounding competencies as required by department. PHYSICAL DEMAND During the course of daily tasks, employee is required to walk, sit, stand and crouch. Must regularly lift/move up to 10 lbs., frequently life/move up to 25 lbs., and occasionally lift/move up to 50 lbs. YNHHS Requisition ID 122098 |
