Reports to: Program Manager, SMC3
Status: Full-time, non-Exempt
Location: Santa Monica Old Firehouse
Schedule: Monday- Friday 8:00am- 4:30pm

Summary
The Santa Monica C3 Clinical Case Manager will perform a variety of tasks to assist the most vulnerable and chronically homeless individuals in their recovery from substances and in regaining control over their lives. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of Santa Monica C3 is to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Clinical Case Manager will engage homeless clients in the field and have primary responsibility in an interdisciplinary team for assessing and referring clients' mental health status, as well as providing general case management services.

Essential Duties and Responsibilities



  1. Work as part of an interdisciplinary team to engage and provide services to homeless individuals.
  2. Interface with partners, including community health providers, the business community, and social service/housing providers to reduce the number of homeless people living on the streets.
  3. Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition.
  4. Serve as the team's mental health consultant and mental health care provider.
  5. Work as part of a larger team composed of all three interdisciplinary teams.
  6. Attend daily team meetings.
  7. Work collaboratively as a team to identify appropriate services taking into account each individual's physical and cognitive needs.
  8. Work collaboratively with public, private, and nonprofit sector groups to provide services; e.g., mobile info stations, public kiosks, hygiene centers, a welcome center, sobering center, etc.
  9. Appropriately chart client contacts.


Qualifications



  1. Ph.D., Psy.D., MSW, or MFT; hours may be available for this position for unlicensed candidates;
  2. Registered and in good standing with the appropriate licensing board
  3. Strong clinical skills and experience
  4. Highly detail-oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
  5. Proficient in Microsoft Word, Outlook, Excel, and database applications
  6. Able to learn quickly and work effectively with a wide range of constituencies
  7. Passion for working with the population served
  8. Ability to work as part of an interdisciplinary team
  9. Able to obtain and maintain CPR/1st Aid certification
  10. Preferred applicants will have experience working with:

    1. those living with co-occurring disorders and experiencing homelessness
    2. individuals with serious mental illness and substance addiction
    3. a social service agency that provides outreach and engagement services
