The purpose of the Clinical Social Worker 3 (MSW) position is to provide both direct patient care to patients and families and assist in the coordination of team delivered services. The Clinical Social Worker 3 will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities. Licensure, Certification, or Registration Requirements for Hire: This licensed clinical social work role will within several interdisciplinary teams in the ambulatory clinics treating adults living with neurological movement disorders including Huntington's Disease, Parkinson's, PSP, Alzheimer's Disease and related dementias, MSA, and ataxia. Many of these clinics are designated Centers of Excellence. This clinical social worker with meet directly with patients and their caregivers to address complex case management needs, engage in advance care planning discussions, and/or provide supportive counseling. This employee will meet with patients face to face in clinic with their team. Additionally this team member will job a team of social workers who support various clinics throughout the Central Virginia area. Work hours are normal business hours, Mon-Fri, major holidays off with PTO, no on call or weekend expectation. Business casual attire is expected.
