Description

The Lutheran Home: Hope Residence is a 52-bed intermediate care facility serving individuals with developmental and intellectual disabilities. Our vision is to share eternal hope, provide hope and inspire hope.

**Hope Residence is searching for Full-Time or Part-Time PM Shift Nurse**

**Weekends only options also available**

Wage Rages:

LPN: starting at $23.95

RN: starting at $30.83

Offer Based on Years of Experience

**Up to $3000 hiring incentive bonus**

As a Nurse at Hope Residence, you will be responsible for delivering high-quality, person-centered care to our residents. You will collaborate with other healthcare professionals, provide clinical oversight for care services, and ensure compliance with state regulations.

Primary Responsibilities Include but not limited too:



  • Conduct initial and ongoing assessments of residents to determine health status and care needs.
  • Develop and update individualized care plans based on residents' medical conditions and personal preferences.
  • Monitor residents for changes in health conditions, providing timely intervention as necessary.
  • Maintain accurate and timely documentation of care, assessments, and interventions.
  • Communicate effectively with residents, families, and healthcare providers regarding care plans and medical needs.
  • Participate in care conferences and provide education to residents and their families on health and wellness topics.


Qualifications:



  • Current and active RN or LPN license in the state of Minnesota.
  • Experience working with individuals with developmental disabilities (preferred but not required)


  • Promote a supportive, person-centered care environment that honors resident dignity and independence.
