Master's Social Worker

Join our team which is committed to the delivery of the highest quality healthcare service. We are seeking a full-time Master's Social Worker who is self-motivated, energetic, and a take-charge individual.

Job Responsibilities:

  1. Conduct thorough psycho-social assessments and develop intervention plans for patients and families. Counsel and advocate on behalf of patients facing domestic violence, protective and ongoing services for special needs children and adults, guardianship, unplanned pregnancies, paternity issues, mental health, or AODA.
  2. Collaborate internally as part of an interdisciplinary team of Providers, Nurses, Behavioral Health Providers, Health Educators, and Case Managers to stabilize crises, improve patient functioning, link clients to services, and coordinate with external community resources.
  3. Maintain a referral network of local agencies and link clients to such agencies.
  4. Maintain accurate documentation in patient charts and submit statistical reports.
  5. Supervise and mentor uncredentialed staff; provide case consultation and review.
  6. Develop policies and service delivery protocols for programs/grants as required.
  7. Conduct workshops for patients and staff on relevant topics.
  8. Represent the clinic in meetings, conferences, and coalitions and actively advocate to end the root causes of poverty, mental health, housing, and health care access.



Qualifications:

  1. MSW (Master of Social Work) degree.
  2. Must hold a current Social Work license.
  3. A minimum of two years of experience in the social services field, working with individuals and families of diverse multicultural and economic backgrounds, is required.
  4. Excellent oral and written communication skills and the ability to prepare reports.
  5. Knowledge of human development, DSMIV mental health diagnoses, crisis intervention strategies, and problem-solving skills.
  6. Familiarity with basic legal principles related to Social Work such as mandated reporting, adoption, guardianship, custody, paternity, emergency detentions, etc.
  7. Comply with licensure and continuing education requirements for the profession.
  8. Valid Wisconsin driver's license, use of an automobile, and current auto insurance.
  9. Bilingual (Spanish/English) required.
