GENERAL SUMMARY: Assists the Patient Placement Coordinator with the multifaceted processes in which patients, newly diagnosed, transferring from an unaffiliated dialysis provider, or visiting on a transient basis to the area, are placed into one of the multiple locations and modalities of Greenfield Health Systems (GHS). PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Works closely with Patient Support Programs Coordinator to provide exceptional
customer services to the renal population, health system and GHS referral sources. - Works closely with the Patient Support Programs Coordinator and GHS Billing
Services Supervisor to verify and trouble-shoot potential medical insurance concerns. - In the absence of the Patient Placement Coordinator, secures placement and
responds to inquiries from discharge planners, physicians, unit staff, administrative staff, and external customers regarding referrals and services within GHS. - Reviews medical records for necessary documentation of patient acuity and special
needs. - Maintain relationships with other health systems and independent nephrology groups.
- Identifies and tracks the needs of transfer and transient patients and notifies the
receiving unit of these needs. Reports to Social Work Manager. - Communicates with internal and external customers regarding problems encountered
with GHS in such areas as patient and physician access to services, complaints, etc. - Promotes patient safety by providing initial medical records to facility staff
electronically for EMR filing. - Provides documentation to the Patient Support Programs Coordinator and insurance
analyst regarding the insurance status of incoming patients. - Creates and maintains transient patient files and corporate reports.
- Maintains/updates files and corporate reports on internal and external transfer
patients. - Collects, organizes, and maintains medical records and statistical data.
- Secures a valid Henry Ford Health medical record number for placement into a GHS
facility. - Assists Coordinator in marketing services to internal and external
physicians/customers. - As part of a larger team, in conjunction with other disciplines analyzes CKD education
along a continuum, to anticipate programmatic needs and contributes toward educational initiatives. - Links patients and families to system and community resources; functioning as a
patient advocate when necessary. - Participates in professional development activities, such as attending relevant
conferences, professional memberships, and professional reading. - Manages, responds, and maintains Careport communication to ensure timely patient
admissions communication. - Demonstrates willingness and ability to function consistently as a team member,
positively contributing daily to the goals and objectives of the dialysis program as appropriate to the scope of the position. - Demonstrates willingness and ability to work effectively in a complex and changing
environment. - Demonstrates willingness and ability to effectively relate in an organizationally
acceptable manner to team members, patients, and other customers in a diverse environment.
EDUCATION/EXPERIENCE REQUIRED:
- Bachelor's Degree preferred.
- Minimum of one year experience in health care environment required. Dialysis
experience preferred. - Ability to analyze pertinent patient information to place patients into Greenfield
Health Systems with minimal delay. - Interpersonal skills necessary to work with a wide range of patients, families, and
multidisciplinary hospital staff. - Computer and IT skills sufficient to access and perform word processing, Excel,
EPIC, QCS and ECIN/Allscripts. - Organizational skills required to manage a high-volume workload.
