Job Title : Licensed Practical Nurse

Location : Charleston, SC 29401

Duration : 12 Months to Long Term

Shift: Work Schedule: 5 Shifts Available, 5x8 Hours (40 Hours/Week)

  • 1st: 7AM - 3:30PM
  • 2nd: 7:30AM - 4PM
  • 3rd: 8AM - 4:30PM
  • 4th: 8:30AM - 5PM
  • 5th: 9AM - 5:30PM



Job Description:

  • Contractor will ensure Contractor personnel provide appropriate, effective interventions to individual patient's needs according to established facility policies, safety standards and procedures as described at the orientations and trainings conducted by Charleston Nurse Education.
  • Any Veterans Health Administration (VHA) employee, volunteer or contractor who works in VHA facilities, visits VHA facilities or provides direct care to those we serve are required to be vaccinated. The contractor employees must provide a copy of their and or provide exemptions (religious or medical) approved by the contractor not government
  • Contractor will ensure that Contract LPN provides skilled nursing care to patients in Acute Care, Primary Care and Specialty Clinics in Charleston and VA Clinics (CBOC's). He/she may perform those approved Medical Center procedures that are described.
  • The LPN demonstrates increasingly more independence in his/her management of patient care and demonstrates competence in caring for adult psychiatric, medical, and geriatric patients. Under the supervision of the nurse manager, the LPN works closely with others as needed in the Specialty Clinics and Primary Care setting.
  • The LPN functions both dependently and independently with guidance and direction from the Nurse Manager/designee.
  • Contractor nurses may be utilized as needed in the Acute Care Medical/Surgical areas, in ambulatory/outpatient care clinics, and outpatient lab.
  • Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or
  • policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD/DO
  • Demonstrated ability to serve as a responsible member of the nursing team and interact [in an appropriate manner] with patients, family members, professional and [other] supportive personnel [involved in the delivery of] patient care, incorporating acceptable, established customer service standards into practice


Education Requirements:

  • Required to be a graduate of a state approved school of practical/vocational nursing.
  • Med/Surg- minimum 2 years current experience (must have worked within the last six months) in a medical/surgical unit. Agency licensed nurses assigned to Charleston VA Med/Surg unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Long Term Care/Nursing Home - minimum 2 years current experience and, (must have worked within the last six months) in a long-term care setting. Agency licensed nurses assigned to Charleston VA Long Term Care unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Psychiatric/Mental Health - Minimum 2 years current experience (must have worked within the last six months) experience in an inpatient psychiatric unit. Agency licensed nurses assigned to Charleston VA Mental Health Psychiatric unit can expect to work 12-hour shifts, with alternating weekend shifts preferred.
  • Primary Care/Specialty Clinics - Minimum 2 years current experience and (must have worked within the last six months) in a primary care setting. Agency licensed nurses assigned to Charleston VA Primary Care/Specialty Clinics unit can expect to work 8-10-hour shifts.
  • May be required to work with COVID 19 positive patients and Patients Under Investigation (PUI), for COVID 19


Qualifications:

  • Personnel assigned by the Contractor to perform the services covered by this Contract shall be currently certified or licensed, as appropriate, in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • All certifications and licenses held by the personnel working on this Contract shall be fully valid and without restriction.
  • The qualifications of such personnel shall also be subject to review and approval by the CHARLESTON Associate Director for Nursing and Patient Care Services and the facility's credentialing office.
  • The government specifically reserves the right to reject any of the Contractor's personnel and refuse them permission to provide services under this Contract


Hours of Operation:

  • The Contractor shall be required to furnish such services on national holidays and during off-duty hours. The Contractor will ensure responsibility for all employees reporting to designated unit as scheduled timely and that each contract employee document timecards accurately reflecting working hours.
  • If attendance requirements are not met, or if fraud/falsifying documents/timecards are discovered, may result in removal from the assignment/contract
  • Work hours are those hours in which a contractor's employee is physically present at the Medical Center in performance of the required services. Normal work hours for Nursing services typically include
  • Specialty/PACT Clinics: Day Shift/Monday through Fridays no weekends no holidays.


Required Documents:

  • Current unrestricted LPN license from any state (United States).
  • Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certificate accredited by the American Heart Association.
  • Covid-19 Vaccination Card or Exemption Form/Letter.
