The Clinical Case Manager provides comprehensive case management for acutely ill psychiatric and substance abusing patients. The case manager completes assessments and works collaboratively with the treatment team to develop treatment and discharge recommendations. This role involves both individual and family work, case management, utilization review, and discharge planning.
