FlexStaff is seeking an Assistant Director of Clinical Services to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. The office is located in Amityville, NY.

Must have a NYS RN License. Additionally, an ideal candidate would have a nursing degree, and completed NYSED-approved infection control coursework; minimum of two to three (2 - 3) years of Clinical Staff Supervisory experience, as well as experience with CHHA, preferred.

This is a permanent, full-time job. Schedule: Monday - Friday 8:30 AM - 5:30 PM, 40 hours per week.

In this role you will be responsible for supervising all day-to-day operations of the Interdisciplinary teams for the LHCSA along with coordination of the multi-disciplinary integrated care in the community, Day Health Center, Diagnostic & Treatment Center.

Responsibilities include, but not limited to:



  • Provide oversight and support to site-based managers and clinical staff, including various departments like rehab, nutrition, social work, and home services.
  • Lead Care Planning meetings, ensure timely follow-ups, and mediate care treatment disagreements.
  • Allocate resources and ensure adequate team coverage.
  • Develop and implement personalized care plans to improve participant satisfaction and outcomes.
  • Oversee service provision affecting participant care and ensure timely care plan completion.
  • Implement training and directives for team development.
  • Collect and review operational data to ensure quality and cost-effective participant care.
  • Participate in quality assurance and compliance activities.
  • Collaborate with clinical departments to enhance care delivery processes.
  • Address and resolve customer issues and grievances in coordination with other departments.
  • Ensure complete and accurate participant documentation.
  • Conduct timely Level 2 investigations.
  • Review participant charts and address deficiencies.
  • Participate in special projects and adhere to scope and competencies set by the PACE organization.
