Job Title: Resource Specialist

Job Location: Region D (Hillsborough, Hardee, Highlands, Manatee, Polk County) - member facing

Job Duration: 3 Months (Potential to Extend/Conversion)

Mon-Fri 8 am-5 pm

Responsibilities: This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member. Compliance with care management policies/procedures including outreach and documentation expectations. Individuals will be audited on a monthly basis to ensure charts are in compliance.
Internal/External Groups with which the Candidate will interface:	Internal- Sunshine Health Care Management Department, Utilization Management, Medical Directors, Provider Engagement External- Community Based Care agencies, Managing Entities, DCF, Agency for Healthcare Administration, Providers
Required Skills/Experience:		Preferred Skills/ Experience:
Pediatric case management experience	1.	Health plan experience
Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.	2.	Foster care and/or adoption experience
