Job Description:

Responsibilities

The Office of Public Health Practice (OPHP) coordinates the New York State's Health Improvement Plan known as thePrevention Agenda. The Prevention Agenda is the blueprint for state and local action to improve the health and well-being of all New Yorkers and promote health equity across populations who experience disparities. The Prevention Agendaidentifies public health priorities for improving the health of New Yorkers, sets measurable objectives, and calls on hospitals and community partners to work together to determine and address their local priorities.

In partnership with more than 100 organizations across the state, the Senior Health Program Coordinator will assist in the coordination of for a collaborative review and revision of the Prevention Agenda framework, implementation, and evaluation. Responsibilities include assisting in the coordination of a state level health assessment; contributing toward the development of guidance for Local Health Departments (LHDs) and non-profit hospitals to complete their health improvement plans; providing support for reviewing community health assessments and improvement plans; and providing technical assistance.

Other appropriate related duties as assigned.

The Senior Health Program Coordinator will serve as part of a team to support the planning, implementation, monitoring, and evaluation of the Prevention Agenda. Responsibilities include:

  • Contributing toward the development of guidance documents and resources to aid LHDs and hospitals in Community Health Assessments (CHA), Community Health Improvement Plans (CHIP) and Community Service Plans (CSP).

  • Contributing to the planning and development of standard operating procedures and guidance for future Prevention Agenda cycles.

  • Assisting in the coordination with NYSDOH programs and external partners to track and help monitor progress of the Prevention Agenda goals and objectives.

  • Contributing toward developing guidance documents, resources, and providing technical assistance to counties and LHDs through various stages of the Prevention Agenda.

  • Assisting in the coordination of planning meetings to support the implementation of the Prevention Agenda.

  • Collaborating with representatives of other State agencies and partners to review and provide support toward the revision of the Prevention Agenda.

  • Contributing to the review and summary of information from the local CHAs, CHIPs, and CSPs submitted by LHDs and hospitals in the State.

Minimum Qualifications

Bachelor's degree in a related field and two years of experience contributing to the coordination of program activities in a public health, health, health regulatory, or human services related program; OR an Associate's degree in a related field and four years of such experience; OR six years of such experience. A Master's degree in a related field may substitute for one year of experience.

Preferred Qualifications

  • At least one-year experience in program planning, development, implementation, and evaluation.

  • Demonstrated project management experience.

  • Demonstrated experience working with diverse audiences and leading a collaborative process.

  • Experience collecting and interpreting data and creating reports.

  • Demonstrated experience providing technical assistance in relation to the interpretation of regulations, policies, and/or guidance.

  • Demonstrated experience preparing and conducting presentations.

  • Experience creating standard operating procedures.

  • Proficiency with Microsoft related applications such as Word, Excel, and PowerPoint.

  • Proficiency with SurveyMonkey or other similar platforms.

Conditions of Employment

Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.

Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.

Travel, up to 25% of the time, will be required.

The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire. Telecommuting will be available.
