Position Description
Employees in this classification are responsible for providing social services to patients and their families by assisting them with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Persons in this position work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem. They also serve as a resource referral service in advising patients and their families about available community resources.

Major/Essential Functions
  • Serve as the primary point of contact for patients and their families, assisting them in understanding and accessing healthcare services
  • Provide guidance and support to patients in scheduling all appointments for patients and accessing healthcare services
  • Conduct virtual visits/phone visits with patients
  • Enter all information in EHR and send information about sessions to billing
  • Answer questions in person and over the phone about our programs, such as the Memory Care Clinic and others
  • Send all documentation to the patient's primary care physician and neurologist
  • Confirm and enter referrals for patients
  • Collaborate with healthcare providers, social workers, and other members of the healthcare team to coordinate care and address any barriers to access
  • Database Management: Maintain accurate and up-to-date records of patient interactions and services provided


Required Qualifications
Education:
A minimum of a Bachelor's degree in Social Work from an accredited program by the Council on Social Work Education.
License/Certification:
Licensed or eligible for license as a Social Worker by the Council on Social Work Education.
