Job Summary

The Home Care Services (HCS) RN Navigator provides clinical review and care coordination for patients being referred to and/or receiving care from a home care program; including home health, hospice, and/or palliative care. HCS RN Navigator responsibilities include patient triage, scheduling, and ensuring timely delivery of care.

Qualifications

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR Certification
  • Graduate of an accredited RN program, BSN, or enrolled in a BSN program
  • Two (2) years of recent home health and/or hospice experience, with at least one (1) year of clinical coordination/navigation in home care.
  • Advanced clinical knowledge in a variety of modalities, or signi cant expertise in a specialty area. Advanced ability to assess patient needs and make appropriate referrals for specialized support services and programs.
  • Advanced skills in assessing patient's grievances and locating appropriate resources to get issues resolved. Advanced skills to intervene and assist with patient's questions or concerns about treatment plan.
  • Advanced patient care skills, working with a high volume and high expectations customer base. Strong ability to support patients with a wide variety of clinical and non-clinical needs.
  • Advanced skills and expertise working in a complex, multi-divisional environment, with proven ability to initiate projects and lead teams, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule.
  • Skills to perform in-depth chart analysis to provide accurate information in a concise, informative manner.
  • Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software.
  • Knowledge of care/services provided in differing levels of care, i.e., ambulatory, and institutional settings, community-based resources, SNF, Home Health, Hospice, and Palliative Care.
  • Analytical skills to identify and obtain pertinent data from multiple sources and work with culturally diverse patients and families with multiple needs or disabilities.
  • Ability to make independent judgments based on clinical knowledge.
  • Ability to work with all levels of staff, both inter- and intra-departmental to create a cooperative relationship and positive work environment.
  • Effective time management skills to accommodate special projects, deadlines, and priorities.

Preferred Qualifications

  • Certified in Hospice and Palliative Care, Case Management, OASIS, or related field
  • One (1) year of acute care, hospital experience preferred.
  • Experience in a role demonstrating use of superior communication skills including therapeutic listening, reflective listening, and teach-back.
  • Training in motivational interviewing

Department Overview

Home Care Services is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Palliative Care. Our administrative team, Clinical Operations, supports all three programs and this position would be a part of that team.

Department Specific Job Scope

The HCS RN Navigator will serve as liaison between patients, healthcare professionals and community-based organizations and act as an expert resource person for all Home Care needs as it relates referral processing and navigating patient care coordination needs. Using a high degree of knowledge of HCS programs, the HCS RN Navigator will utilize the expertise in areas including but not limited to, problem-solving that frequently requires analysis of unique issues, and problems without precedent and/or structure. May manage patient care and program situations that include formulating strategies and administering policies, processes, and resources; continuing analysis of unique challenges related to home care; and coordinating care needs across the health care continuum. The HCS RN Navigator functions with a high degree of autonomy. This position is a collaborative member of the HCS Clinical Operations team. This position will work collaboratively with UC Davis Health departments including but not limited to, Clinical Case Management, Transitions of Care, Managed Care and other Health Plans, Home Care Collaborative agencies, skilled nursing facilities, Durable Medical Equipment and other outpatient providers, and other health care professionals and agencies to ensure a smooth transition and improve delivery of services to home care patients. This position is performed onsite.

Work Environment

UC Davis is a smoke and tobacco free campus effective January 1, 2014.
