Now Hiring: Join Central City Concern as a Shelter-Based Clinical Nurse

Location: Central City Concern - Hansen Clinic (Banfield Motel Shelter - 1525 NE 37th Avenue, Portland 97232)

Compensation: $44.05 - $57.27/hour (Dependent on years of formal RN experience + internal equity)

MINIMUM QUALIFICATIONS (REQUIREMENTS)



  1. Required: Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course.
  2. Required: Current RN license in the State of Oregon.
  3. Required: Must have experience working with high-risk participants.



PREFERRED QUALIFICATIONS (GREAT TO HAVE, BUT NOT REQUIRED)



  1. Preferred: Wound care experience.
  2. Preferred: Valid driver's license preferred. If required to drive, must pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. If required to drive, must pass an initial driver's training within 60 days of being an approved driver and continued recertification training.



ESSENTIAL DUTIES AND RESPONSIBILITIES:

Ambulatory Care RN Duties:



  1. Use nursing assessment and clinical skills in an integrated primary care setting that includes substance use and mental health treatment.
  2. Participate as a member of the interdisciplinary team to provide integrated patient care.
  3. Act as a champion of clinical best practices from a nursing perspective.
  4. Attend CCC trainings and meetings as appropriate to job scope.
  5. Assess and triage patients in-person and/or by phone.
  6. Provide nurse visits and/or provider/nurse co-visits with utilization of nursing protocols. Visits include nursing assessment, evaluation and treatment with provider consultation of acute and chronic health conditions as needed.
  7. Document in electronic health record all nursing assessments, interventions, plan and educational activities in a timely manner.
  8. Visits may include (but are not limited to) on demand health assessment and chronic disease management, wound care, testing, screening, foot care, focused health visits.
  9. Provide focused and literate friendly patient education.
  10. Assist clients with medication adherence coordinating with pharmacy and providers as needed.
  11. Provide instruction, training, and health information on self-care and health maintenance as per the client's care plan
  12. Administer medications per provider order, with appropriate documentation and in conjunction with regional RN standing orders.
  13. Respond to urgent in-person and telephonic situations.
  14. Provide clinical and operational oversight to medical assistants and/or other clinical and support staff.
  15. Lead problem solving and Collaboration with team members across Central City Concern, and outside of the organization to maximize PCP engagement.
  16. Support high-quality patient experience and address concerns of patients and external partners related to visit experience.
  17. Provide medication and treatment monitoring, in partnership with pharmacy and provider, in accordance with client's care plan.
  18. Provide appropriate health care screenings as indicated to address care gaps
  19. Support completion of referrals and coordinate care.
  20. Support community partners in furthering understanding of the medical and psychosocial sequelae of homelessness and the systems and processes used to help complex and medically vulnerable participants secure and maintain stable housing, healthcare, and other social services.
  21. Participate in compliance, quality assurance and quality improvement activities as directed.
  22. Work as a member of the CCC multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services.
  23. Actively participate in the integration of services between CCC and community partners and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  24. Proactively identify opportunities for improvement within CCC workflows and logistics. Participate in review of Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  25. Build understanding of and appreciation for diversity among CCC clients and support their connection to culturally responsive and culturally specific services as requested.



Hansen RN Duties:



  1. Provide leadership for clinic operations in conjunction with primary care manager and designated clinic provider.
  2. Evaluate clients for medical stability as needed upon admission to shelter and ongoing to ensure safety in shelter (including appropriate hospital discharge planning).
  3. Participate in and facilitate daily and weekly care coordination with shelter and wellness staff.
  4. Provide TPI shelter staff basic medical education and support in service to clients health and wellness.
  5. Support, integrate and partner with TPI and community PCP to assist clients to engage with proactive health care.
  6. Maintain medicines, point of care tests and medical and lab equipment and records in compliance with regularly requirements.
  7. Lead, maintain and improve work flows and clinic organization.
  8. Triage client schedules in conjunction with Hansen staff and provider according to health acuity and treatments needed.
  9. Coordinate and communicate clinic staffing with TPI and Hansen staff to provide predictable and low barrier clinician access.
  10. Coordinate pharmacy deliveries from CCC pharmacy to shelter clients as needed.
  11. Coordinate with client access team on workflows that streamline registration and clinic access.
  12. Maintain lab and specimen collection and processing including autoclave coordination
  13. Occasional phlebotomy.



Administrative:




  1. Responsible for the oversight of the clinic space, including timely ordering of supplies, recognition and facilitation of equipment maintenance, and ensuring a clean and organized clinical care area.
  2. Attend all mandatory CCC trainings in a timely manner.
  3. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  4. Perform other duties as assigned.

  6. Working knowledge of principles, practices and procedures of outpatient care and programs and services available to participants.
  7. Ability to be flexible and to coordinate and relate effectively with members of several interdisciplinary teams, participants, supervisors and co-workers.
  8. Ability to provide leadership
  9. Ability to work autonomously and with confidence to make decisions and problem solve in the moment and with ongoing client health and wellness.
  10. Ability to work independently, prioritize the workload, problem-solve, and complete tasks in a timely fashion.
  11. Excellent oral and written communication skills.
  12. Ability to maintain strict standards of confidentiality with regard to patient information.
  13. Ability to maintain a calm professional demeanor and make independent judgments.
  14. Ability to maintain composure and provide therapeutic communication and care to participants who have varying social and emotional communication skills, and/or under the influence of drugs or alcohol.
  15. Ability to be a detail oriented, flexible, dependable, positive team player.
