Wage Rage: $25-$28 Hourly, depending on experience
Location: St. Louis Park
Schedule: Monday-Friday 8am-5pm

The Community Intake Coordinator is accountable for receiving, reviewing, reconciling, and processing all incoming private pay referrals, including receiving referral via fax, phone, or email; determining if referral can be accepted based on resource for payment, service area, and scope of nursing and/or caregiving needs; review referrals for necessary components and determine missing components; reconcile missing referral information through communication and coordination with TCUs, Primary MD clinics, hospitals, etc; process and oversee the processing of referrals into EMR; coordinate referral flow and capacity with department leadership team; identify additional needs and coordinate to meet client needs. This position is also responsible for assisting with client record management including faxing and tracking physician orders and maintaining client information in the EMR.

Position Functions and Responsibilities



  • Manage Community Home Care intake from receipt of referral to hand-off to RN Case Manager and Staffing Team
  • Receive referrals via phone, fax, or email.
  • Review referrals for required components and additional gaps to meet client needs.
  • Reconciles missing components of referrals through follow-up/coordination with providers
  • Coordinate with Director of Community Home Care Services, Clinical Manager, and RN Case Managers to manage referral and achieve timely service delivery within company standards.
  • Maintain accurate EMR environment to compliance standards.
  • Responsible for converting calls to face-to-face meetings in order to hit the conversion target.
  • Follows up on pending referrals according to department standards.
  • Follows up and tracks clients admitted to hospital or transitional care units.
  • Follows up and tracks clients who are on hold and contacts clients/family members to resume services.
  • Schedules RN Case Managers for in-person consultations and admissions.
  • Tracks and schedules required reassessments with the RN Case Managers
  • Assists with client record management including:


  • Faxing and tracking physician orders
  • Uploading physician orders and clinical documentation to the client's EMR



    • Entering and updating client information into the EMR


  • Maintains tracking of clients hospitalized or in a skilled nursing facility and updates RN Case Managers accordingly.
  • Manages incoming/outgoing faxes.
  • Assists Operations Manager with the billing process
  • Submit client invoices and requested documentation to long-term care insurance on a monthly and as needed basis.
  • Provides weekly and monthly intake reports to stakeholders.
  • Other duties as assigned by manager.


Qualifications



  • Minimum of 1-2 years of experience in healthcare
  • Minimum 1 years of experience in home health
  • Experience with referral and/or intake preferred
  • Proficiency with medical terminology
  • Knowledge of market landscape, community resources, payer sources
  • Strong phone skills, Customer Service Focus, Sales awareness
  • Proficient with technology and technological applications: Microsoft Office, Calendaring, Wi-Fi, EMR, Internet Browsing, Printer
  • Customer-focused orientation: in all service activities, listens and responds to what current and potential customers want
