PACE, a member of Empath Health is currently seeking an Occupational Therapist to join our team!
The Occupational Therapist:
- Under the supervision of the PACE Rehab Supervisor, participates as a member of the Initial Care Plan team, completing Home Safety Evaluations for potential new enrollees to the PACE program.
- Participates a member of the Hospital-Short Term Nursing Home Transitions Team as needed to assist with discharge planning for transition from hospital to home, hospital to short term nursing home placement, and/or from short term nursing home stays back to community-based setting.
- Serves as a required member of the Interdisciplinary Team and may attend IDT meetings daily if the assigned OT for one of the IDT teams is on vacation.
- Conducts an in-person initial comprehensive occupational therapy assessment on all Suncoast PACE participants upon enrollment and coordinates with the Interdisciplinary Team on a Plan of Care for each participant.
- Evaluates participant's needs. Follow-up with participants during treatment, explaining precautions to take.
- Supervises Certified Occupational Therapy Assistant in accordance with the FL OT State Practice Act once a plan of care established.
- Constructs or provides orthotic and adaptive equipment as needed.
- Conducts assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety.
- Assists as liaison with Suncoast PACE nursing home and assisted living staff regarding occupational therapy needs of participants who are their residents.
- Evaluates charts and reports on participant's progress, documents occupational therapy notes in the participant's medical record.
Position Requirements
- Graduate of an occupational therapy curriculum.
- Legally authorized, currently licensed, registered, or certified if applicable to practice in the state of Florida.
- Minimum one year of experience working with the frail or elderly population
- Valid CPR certification from the American Red Cross or American Heart Association.
- Intermediate proficiency in MS Word, Excel, PowerPoint, MS Outlook
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide valid FL driver's license and proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
