Overview:

As a Palliative Care and Hospice Coordinator, he/she is responsible for: (1) coordinating the care of patients admitted to the Hospice and Palliative Care program, which will include outpatient clinic, consult service, inpatient hospice unit and outpatient hospice, (2) attending care management rounds and Hospice Interdisciplinary Group meetings (outpatient and inpatient); and other interdisciplinary rounds as needed.(3) the initial assessment of patients for eligibility criteria to the different program components, and concurrent care management reviews, (4) knowing Hospice and Palliative Care practice, philosophy, policies and regulations, (5) working for the accreditation of the Palliative Care Program through Joint Commission certification, (6) ensuring that services provided meet standards of quality and expectations as an outreach of the Medical Center in its ministry to the sick, (7) using Epic computer system effectively and efficiently, and (8) obtaining current knowledge and professional practice of the following: Hospice Medicare and Medicaid regulations Arkansas State Hospice and Palliative Care licensing regulations Conditions of Participation (COPs) for hospice JC and Arkansas State Standards and Inspections Quality Standards and Quality Assessment and Process Improvement (QAPI) within the Palliative Care setting. Qualifications:

  • MINIMUM REQUIRED Education: Bachelor of Science in Nursing (BSN) or BSN degree to be completed within 2 years of employment.
  • Licensure: Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements.
  • Experience: Five years experience in a clinical setting. Knowledge of health care delivery systems and utilization management principles.
  • Experience in long term care, Home Health, Hospice and Palliative Care preferred
