Elwyn seeks a Site Manager to be responsible for ensuring the cleanliness and safety of the program and providing general daily care for program participants. The position assists young adults through geriatric individuals with chronic mental illness in learning essential daily living skills to achieve social, economic, emotional, and personal independence across one or more programs. ESSENTIAL FUNCTIONS:
Maintains Cleanliness of the Site
- Assures Environment of Care standards are met.
- Assists participants in a teaching atmosphere to educate them on maintaining a clean environment through hands-on skill building.
Maintains Safety
- Completes bi-annual FHR Safety Report.
- Maintains a Fire Log, organizes monthly fire drills, and completes self-preservation reports.
- Maintains the Emergency Response Plan by implementing ERP drills, replenishing ERP supplies, and completing ERP reports.
- Coordinates supplies and maintenance of the facility.
- Maintains and disposes of biohazard material and sharps.
Maintains Medication
- Supervises medication as instructed by MAP regulations.
- Supervises medication packing.
- Completes medication logs.
Empowers Participants
- Integrates the PRISM MODEL into everyday practice and clinical intervention. Engages participants through therapeutic rapport, advocacy, and skill-building.
- Implements interventions as prescribed by Individualized Action Plan (IAP).
- Assists participants in a teaching atmosphere to educate them in ADL skills and nutrition through counseling techniques and other individual methods to promote independence.
- Trains and supports participants in communication, social, and recreational skills, and the skills and knowledge required to transition to independent living.
- Encourages constructive activities within the residence and the community by implementing recreational and educational programs to enhance participants' quality of life.
- Assists participants in budgeting, maintaining, and monitoring funds.
- Facilitates and supports participants' maximal integration into the community by providing services responsive to their preferences and needs, focusing on recovery.
- Maintains caseload as requested and completes documentation as required in accordance with FHR, CARF, and DMH Rehab option standards.
Office Administration
- Supports staff as requested in managing participant records by filing and purging.
- Provides input to staff evaluations as requested.
- Assists in participant record management by documenting progress notes.
- Performs other duties as assigned.
QUALIFICATIONS, EDUCATION & EXPERIENCE:
- Education: Bachelor's Degree in a related field is preferred, or a High School Diploma or GED with 2-3 years of field experience in a similar position can be substituted for the bachelor's degree requirement.
- Experience: Previous experience in a similar role, particularly in mental health or social services, is highly desirable. Experience with administrative tasks, program coordination, and direct care is beneficial.
- Skills:
- Communication: Excellent verbal and written communication skills to effectively interact with program participants, staff, and external stakeholders.
- Organizational: Strong organizational skills to manage multiple tasks, maintain records, and ensure the smooth operation of the program.
- Problem-Solving: Ability to identify issues and implement solutions to maintain the safety and cleanliness of the site.
- Interpersonal: Strong interpersonal skills to build therapeutic rapport and engage participants in skill-building activities.
- Technical: Proficiency in using computer software for documentation and reporting purposes.
- Certifications/Licenses:
- Current Medication Administration Program (MAP) certification within six months of hire.
- Obtains and maintains current Medication Administration Program (MAP) certification within six months of hire, CPR, and First Aid Certification.
- Must possess a current, valid driver's license in the state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record.
- Additional Qualifications:
- Familiarity with FHR, DPH, and DMH policies, regulations, and standards.
- Event Coordination: Ability to plan, organize, and manage events or programs, ensuring they run smoothly and within budget.
- Safety Management: Knowledge of safety protocols and the ability to implement and monitor safety procedures effectively
