Qualifications EDUCATION Graduate from an accredited College of Pharmacy with a B.S., M.S., or Pharm. D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is highly desired but not required. EXPERIENCE Minimum of three years of experience in a clinical practice setting with demonstrated skills in formulary/access evaluation, drug information, and project management. Experience in leading or participating in medical committees and application of quality improvement techniques is required. LICENSURE Active Connecticut Pharmacist License SPECIAL SKILLS Experience in drug utilization/access review and data analysis (e.g. MS Excel, MS Access) is highly desirable. Excellent written and verbal presentation skills are required. Should be familiar with Change Acceleration Process (CAP) or similar process improvement/change management skills.
