Inova Kellar Center is looking for a dedicated VMAP Kellar Care Navigator to join the team. Care Navigators on the team work directly with families and patients to assist them in accessing behavioral health care. This role will be full-time day shift from Monday - Friday, 8:30 a.m. - 5:00 p.m. Hybrid.

Virginia Mental Health Access Program (VMAP) Kellar Care Navigator Job Responsibilities:

* The Virginia Mental Health Access Program (VMAP) Kellar Care Navigator maintains documentation and data base.
* Works collaboratively with VMAP Regional Hub Teams to ensure success of the program.
* Links PCPs and families to the appropriate behavioral health services and supports based on their specific needs and insurance status.
* Tracks calls, referrals and any other data required by leadership. Provides crisis intervention and behavior management services. Provides daily check-ins and various group counseling interventions.
* Develops and maintains relationships with intake coordinators at community mental health agencies, healthcare clinics, and organizations.

Additional Requirements:

* Work schedule: Monday - Friday, 8:30 a.m. - 5:00 p.m. Hybrid
* Education: High School or GED
* Experience: Two years' experience working with Community Social Services or Education System.
* Preferred) working with adolescents, community health work or family peer support and Spanish speaking.
