JOB DESCRIPTION:

SUMMARY OF POSITION

Coordinates all referrals and placements including participating on a multidisciplinary team to ensure the success of the placement. Manages all child referrals including on call at night. Evaluates the referral and determines if there is an appropriate placement for the child. Determines if an exception is needed and completes the required paperwork. Ensures compliance with statutes and guidelines and that all client files are accurate and complete.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Serves as the initial point of contact to receive referrals from the child welfare system. Answers all referral calls, emails and texts within one hour of receipt as required.
  • Evaluates the needs of the child indicated in the request. Matches the child to an appropriate foster home according to their needs level and the ability of the foster family to meet those needs and willingness of the family to accept the placement. Determines if an exception is needed and completes the required paperwork.
  • Coordinate admission and placement of child into foster home by working as a member of a multidisciplinary team to obtain and maintain essential elements of the child's clinical record, including but not limited to: intake documents and required supporting documents, reporting, corresponding with external agents, and maintaining compliance with licensing and contract standards. Emails intake packet to the child welfare system within 24 hours of admission and ensures all documents are returned fully completed and signed.
  • Updates the electronic medical record system including entering the child in the system, managing their placement record, maintaining and updating the record as needed, and coordinating with the supervisors and staff who will be working with the child to ensure a smooth transition to the foster home.
  • Responsible for maintaining admission records for cost report that is submitted to DCS annually.
  • Maintains database of all foster families and their availability for placements and preferences and/or licensing level they have on those placements. Ensures ratios are accurate for compliance.
  • Attends staffings to share concerns or challenges and provide feedback on processes and placements.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of Microsoft Office.
  • Knowledge of state statues and guidelines for Foster Care.

QUALIFICATIONS

Education Level

Bachelor's degree required.

Years of Experience

3 years of experience.

At least 1 year of LCPA experience.

Certification/Licensure

N/A
