Overview

Provides general medical care and treatment to patients in medical facility such as clinic, health center, or public health agency, under direction of Physician, by performing the following duties.

Mercyhealth, an award-winning fully integrated health care delivery organization based in Wisconsin and Illinois, is seeking an experienced Pediatric Nurse Practitioner for a rewarding and fulfilling opportunity to join our busy Pediatric Clinic Rockford, IL. This position provides general medical assessment, evaluation and treatment of patients accessing the Pediatric Clinic for a health condition. Position is under direction of Pediatric Care Physician partner.

Responsibilities

  • Assess patient data and perform interview, physical examination and begin discussing goals of care within prescribed guidelines and instructions of the Pediatric Care Physician.
  • Maintain timely consult schedule.
  • Provide patient and family education to assist Pediatric patients and their family members to understand role of Pediatric Care in their current health situation.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required.
  • Record physical findings, and formulate plan and prognosis, based on patient condition.
  • Coordinate care with Pediatric Care Physician and other health professionals; preparing comprehensive patient care plan.
  • Discuss health care plan and goals of care with Physician partner.
  • Consult with partnering physician and refer to specialized health resources for treatment and evaluation as needed.
  • Assists in developing Pediatric Care protocols and processes.

Education and Experience

  • Graduate of an accredited nursing program.
  • Graduate of an accredited masters of nursing program.
  • Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.

Certification/Licensure

  • PNP certification preferred
  • Hold valid RN license in state of practice.
  • Hold valid APNP license in state of practice.
  • Approved CPR or BLS certification required within 90 days (Other certification per unit requirements: ACLS, PALS, PEARS, NRP)
