Summary:

The Care Coordinator assists members/patients by providing support, education, and assistance in the prevention and/or maintenance of their health and wellness state. This role monitors members'/patients' treatment plans, educates them about resources available to support compliance with the plan of care, connects them with healthcare providers, and evaluates their progress. They work with administration, Care Managers, members/patients, and their family members/patients to reach healthcare goals and keep the lines of communication open.


Responsibilities:


  • Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
  • Serves as a resource contact and information/education source to members/patients, families, providers, and/or staff.
  • Provide telephonic follow-up with members/patients for Care Management services to include post-hospital discharge and chronic condition management gap closure.
  • Completes assigned tasks related to care coordination.
  • Generates tasks to the assigned Care Managers.
  • Defines and directs members/patients and/or families to appropriate resources.
  • Educates self and communicates to others about community resources available to assist in the treatment plan.
  • As directed and supervised by the Supervisor, assists in the attainment of members'/patients' goals.
  • Work with members/patients, physicians, and other care providers to meet the member's/patient's individual needs based on protocols and referrals.
  • Call the identified member/patient to offer a review of the program.
  • Inform the member/patient how to access resources.
  • Assist in the arrangement of Provider follow-up appointments and delivery of care.
  • Mail approved educational and informational materials.
  • Assist in the performance of calls to members/patients post-discharge from inpatient or other levels of care.
  • Support the monitoring of interventions in the Plan of Care.
  • Conducts communications in a professional manner.
  • Assisting in the arrangement of Provider follow-up appointments and delivery of care,
  • Mailing of approved educational and informational materials,
  • Assisting in the performance of calls to members/patients post discharge from inpatient or other levels of care,
  • Supporting the monitoring of interventions in the Plan of Care.
  • Conducts communications in a professional manner.
  • Recognizes and adheres to the scope/limitations of job requirements.
  • Contributes to team efforts by accomplishing related results as needed.
  • Demonstrates relationship-building and influential skills.
  • Has outstanding Customer Service and follow-up skills.
  • Has strong organizational and time management skills, able to work with little supervision.
  • Has excellent written/verbal communication skills, especially the ability to communicate telephonically in a professional and effective manner.
  • Has Computer literacy in Microsoft Office Suite (Excel, Word, Outlook, PowerPoint) and Internet navigation.
  • Has the ability to multi-task in a fast-paced environment.


Job Requirements:

Education/Skills


  • High School diploma or equivalent required.
  • Associate's degree or higher in allied health professional field of study preferred.


Experience


  • 1-3 years of experience preferred.
  • Healthcare related experience preferred.


Licenses, Registrations, or Certifications


  • None required.
