The Transplant Outreach Coordinator serves as a liaison and resource to all potential referral sources to the Hume-Lee Transplant Center of the VCUHS with the intent of increasing patient referrals for solid organ transplant evaluation. The Transplant Outreach Coordinator implements marketing strategies that increase referral base and promote the Hume-Lee Transplant brand and initiates relationships and conducts outreach and educational activities and customer service to physicians and their staff. The coordinator acts as a clinical liaison between the referring providers and referred patients and works to increase the referral pool of patients. The outreach coordinator works independently while collaborating with other members of the transplant and marketing teams.

Essential Job Statements

  • Serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered at VCU Health and in the community.

  • Raise awareness at community health fairs, local church events and medical conferences. Contribute to a strategy for increasing living liver donation by educating outside center staff.

  • Coordinate outreach efforts to establish and maintain positive working relationships with key gastroenterologists, hepatologists, and primary care providers.

  • Develop a patient support group, education programs and tools

  • Publicize transplant research opportunities offered at VCU Health, not offered at other transplant centers

  • Promote adoption of EPIC Carelink for community providers to streamline referral process.

  • Manage education of dialysis center personnel to appropriately collect and send monthly blood samples for listed patients.

Patient Population

Demonstrates the knowledge and skills necessary to provide equitable care appropriate to the age of the patients served on their assigned unit.

Demonstrates knowledge and related competencies of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to their age, specific needs and to provide the equitable care needed as described in departmental policies and procedures.

Employment Qualifications

Required Education:

BS in Nursing

Preferred Education:

Licensure/Certification Required:

Current RN licensure in Virginia or eligible or compact state

Licensure/Certification Preferred:

Minimum Qualifications

Years and Type of Required Experience

Min 3 years' experience as an RN, preferably with GI/ Hepatology or liver transplant experience

Working Conditions

Periods of high stress and fluctuating workloads may occur.

General office environment.

Required to car travel to off-site locations, occasionally in adverse weather conditions.
