The Behavioral Health Service Coordinator (LMSW, LCSW, LPC or LMFT) is responsible for performing service coordination activities within their scope of licensure for members with behavioral health, co-occurring behavioral and physical health, and/or substance use disorder needs. The BH Service Coordinator enables Cook Children's Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation.

Duties include: Performs primary service coordination functions of assessment, planning, facilitation, and advocacy; Develops individual service plans (ISPs); Facilitates care transitions, documents service coordination & collaborates and problem solves with other members of an interdisciplinary team.

Required Education, Licensure and Skills:

  • Requires Master's degree in Social Work, Counseling, Behavioral/Mental Health, or related field.

  • Minimum of three to five (3-5) years direct care clinical experience in case management, service coordination, or population health with a broad range of knowledge regarding Serious Mental Illness (SMI) and Serious Emotional Disturbance (SED).

  • Must have valid LMSW, LCSW, LPC or LMFT Licensure in Texas.

  • Knowledge of assisting with complex psychiatric/substance use disorder cases and discharge planning.

  • Service Coordination or Managed Care Organization experience preferred.

  • Experience in motivational interviewing techniques preferred.

  • Oral and written communication; Critical thinking; Organization and time management; Proficiency in Microsoft Office and internet/web navigation Proficiency in navigating Electronic Medical Records and Customer service.

  • Ability to work independently & remotely upon orientation completion

Must exhibit strong skills in the following areas: Experience working with children, adolescents, and adults who are diagnosed with behavioral health disorders and substance use disorders, including but not limited to Bipolar Disorder, Schizophrenia, Major Depression, Borderline Personality Disorder, Oppositional Defiant Disorder, Anxiety Disorder, and other related areas.
