Location: Calmont Operations BuildingDepartment: Behavioral Health CMShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: Additional Information: The Behavioral Health Service Coordinator (LMSW, LCSW, LPC or LMFT) is responsible for performing service coordination activities within their scope of licensure for members with behavioral health, co-occurring behavioral and physical health, and/or substance use disorder needs. The BH Service Coordinator enables Cook Children's Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation. Duties include: Performs primary service coordination functions of assessment, planning, facilitation, and advocacy; Develops individual service plans (ISPs); Facilitates care transitions, documents service coordination & collaborates and problem solves with other members of an interdisciplinary team.
Must exhibit strong skills in the following areas: Experience working with children, adolescents, and adults who are diagnosed with behavioral health disorders and substance use disorders, including but not limited to Bipolar Disorder, Schizophrenia, Major Depression, Borderline Personality Disorder, Oppositional Defiant Disorder, Anxiety Disorder, and other related areas.
