Position Title: Pharmacist 3 Work Location: Remote (preferred TX, MO, FL) Assignment Duration: 12 months Work Schedule: 8 am -6 pm Central time Position Summary: Define and develop standard and custom formularies for assigned plan. Background & Context: Remote data verification and prescriber/patient calls for home infusion, IVIG, and hemophilia orders The team is currently a relatively small one used to supporting each other and working together well. Key Responsibilities: Develop clinical criteria for medications, recommend plan design changes, and clinical programs to be initiated Monitor prior authorization requests Provide clinical support to internal departments and address clinical related questions Ensure appropriate quality controls and initiates opportunities for performance improvement in pharmacy/practice Develop and implement programs designed to impact DUR for both Medicaid and Medicare Develop, implement, and maintain policies and procedures for the pharmacy department Participate in the coordination of the Medicare MTM program Assist case management team with members including clinical rounds presentations Receiving/clarifying prescriptions from provider's office with all necessary information for home infusion including but not limited to: meds prior to infusion, flushes, calculate infusion time, nursing and all necessary supplies/pump. Counseling and providing care plans to patients as well as support for providers and home infusion nurses Expectations: to adapt to changing workflow, prioritizing tasks, can take initiative and work independently while maintain cohesion with the team Providing individualized clinical care to patients with rare disease states Qualification & Experience: Education/Certification: Required: Bachelor's degree in pharmacy; Preferred: IGNS certification Licensure: Required: Active Pharmacist License in TX, MO, or FL. Preferred: [none] Years of experience required: 3+ years of mail order, retail or hospital pharmacy experience OR PharmD Additional qualities to look for: Any kind of IVIG, home infusion or hemophilia experience, or anything that suggest more complex tasks or therapies. Experience with infusion pumps and supplies. Experience with multi state mail order pharmacy.
