Preferred Qualifications: The Manager of Operations and Strategic Initiatives will be responsible for designing, implementing and evaluating regional community health initiatives across the state focused on advancing Diabetes programs and support, as well as building the Community Health Worker workforce as an integral part of Diabetes care. This role will be responsible for grant management, partnerships with community-based organization and connecting clinical care and social care systems.
