Overview
FT Days-7am-5pm (weekend rotation) Pay-$130,000-$150,000 Bonus-$15,000 Develops and oversees the ongoing High Acuity Nursing program, including but not limited to regulatory and training standards, quality metrics, clinical pathways, protocols, skills labs, and competency assessments for various care models (acute, post-acute, chronic care management, etc). Responsible for ensuring delivery of services and appropriate clinical care within the home, employee education, market clinical oversight and ensuring clinical outcomes in collaboration with other functional teams.
Responsibilities
- Develops, leads, and manages the High Acuity Care (HAC) Recovery Care Coordinators across multiple sites within the market in collaboration with Senior Director of Clinical Operations.
- Leads market Recovery Care Coordinators through change and growth opportunities.
- Creates and monitors regulatory and clinical competency/skills assessments.
- Provides direction, quality data, and oversight to the HAC market to ensure clinical outcomes.
- Develops of standards of care, quality metrics, clinical policies, protocols, and procedures in accordance with care standards.
- Meets routinely with internal and external partners to educate, share clinical information, and receive feedback on market performance.
- Collaborates with market implementations team to meet needs related to expansion of new service lines within the market.
- Supports ongoing nursing education as needed, including new employee onboarding, continuing education, and training of market team, in collaboration with the Learning & Development team.
- Assists in improving programs, processes, and technology.
- Performs administrator on-call responsibilities.
- Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on the job training and development opportunities.
- Ensures compliance with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other related duties as assigned.
Qualifications
Required:
- Bachelor of Science in Nursing degree.
- Current, unencumbered license to practice as a registered nurse.
- Five (5+) years of acute care nursing experience.
- One (1+) year of leadership experience.
- Experience building, leading, and executing clinical programs.
Preferred:
- Two (2+) years of experience in nursing leadership, preferably leadership of a diverse clinical team across multiple sites.
- Remote monitoring or telehealth experience.
- Master of Science in Nursing degree.
