STGi is currently seeking a Medical Records/ LVN to provide services at The William R. Courtney Texas State Veterans Home.



The William R. Courtney Texas State Veterans Home is located in Temple, TX. Our staff provides compassionate and specialized care to our nation's veterans. We focus on providing the best experience for residents and their families.



JOB SUMMARY: To support the effective operation of maintaining accurate and complete resident medical records by following company standards.



ESSENTIAL FUNCTIONS:


1. Ensure that all medical record information, including resident PHI (protected health information) is protected and kept confidential.


2. Protect all medical record information from loss, defacing, or destruction before retention period ends.


3. Retrieve/Request medical records promptly upon request by authorized individuals.


4. Identify late, incomplete, and/or inaccurate documentation and report to individuals responsible for completion and accuracy.


5. Assist in ensuring that Medicare patients have timely certifications/re-certifications signed by the attending physician.


6. Process admission and discharge records timely and accurately.


7. Verify diagnoses with Director of Nursing and record on admission record.


8. Audit medical records, as assigned.


9. Receive and file all diagnostic reports promptly and accurately.


10. Ensure all state, federal, and company guidelines are followed regarding medical records.


11. Perform other related duties as directed by his/her supervisor.


12. Scan all paper records into residents' PCC profile.


13. Process all medical record requests with proper approval.


14. Monitor frequency of physician visits and progress notes per regulatory guidelines.

Required Skills

REQUIRED EXPERIENCE AND SKILLS:


One (1) year of Health Information Management experience required. Must have an RHIT, RHIA, or valid Texas or Compact Party State nursing license (RN/LVN/LPN), or valid CNA certification. LTC experience highly preferred.

The physical demands and work environment described herein is a representative of those that generally must be met by a team member to successfully perform the essential functions of this job:


* This position primarily involves standing, walking, twisting, reaching, bending/stooping, pushing and pulling (typical weight of 5 lbs. and maximum weight of 50 lbs., approximately), and lifting (minimum weight of 5 lbs. and maximum weight of 50 lbs., approximately).


* The position requires repetitive finger movements as well as hearing and talking on the phone and in person.


* Must be able to continuously deal effectively with stress created by residents, multiple tasks, noises, interruptions, and work cooperatively as part of the health care team while maintaining a pleasant attitude.
