Summary

The Transfer Center coordinates medical transportation for all four adult hospitals, the childrens hospital, and the free-standing emergency department at Lee Health Coconut Point. The department also includes Hospitalist Access Specialists that assign hospitalists for patient admissions and transfers.

These nurses work closely with radiology and bedside nurses to prepare and schedule imaging, ensuring an efficient flow and prioritization of patients who need to be seen urgently. Potential to work a hybrid schedule.

In 2024 this team of 32 people safely moved 42,297 patients and fielded approximately 25,000 calls per month!

Participates as a member of the Transfer Center (TC) in driving communication and coordination of patient transfers/admissions throughout the hospital system. The position evaluates patient status need and determines the appropriateness and priority of the request based on system specialization and hospital bed availability. The TC Coordinator is the first responder for all communication to and from the TC, including LMHS hospitals(Intra-System), doctor's offices (Direct Admits), external outpatient facilities (Clinics), and external health systems (Inbound transfers). The TC Coordinator must have a strong clinical knowledge and ability to perform rapid triage while working closely with internal hospital management, physicians, and ambulance personnel. The Transport RN will be required to perform intra-system and inter-facility transports of the acute care and or/injured patient as triaged for RN level care. These transports will occur on an ambulance with supporting EMT/Paramedic personnel. This role will require patient assessment, plan and implementation of plan along with evaluation of the patient in collaboration with other health care professionals and physician guidelines. The level of the transport skillsets will need to include but not limited to knowledge of invasive hemodynamic monitoring, critical infusions and ventilator management. Individual in this position must be able to handle a high demand, fast paced work environment with a strong ability to effectively multi-task and prioritize.
