The Pharmacy Medication Transition Technician assists inpatient and outpatient pharmacists with medication history, reconciliation, to ensure that any differences between medications taken before admission and medications ordered upon discharge for a patient are intentional and not accidental. Works to promote an efficient discharge prescription process in collaboration with the outpatient pharmacy.

Additional Information
Department: OP Retail Pharmacy
Job Status: 40 hours per week, eligible for benefits
Shift: Days/evenings (not a set schedule) and rotating weekends.
Shift Times: Monday - Friday, 8am-8pm. Saturday, 8am-5pm and Sunday, 9am-2pm.
Shift Length: Varies between 5-10 hours.
Weekend Requirement: Rotation determined by business need and training.
Holiday Requirement: No holidays at this time.

Eligible Incentives: Relocation assistance available to qualified external applicants.

Salary Information: $23.54 to $35.31 *Pay is dependent on applicant's relevant experience

Duties & Responsibilities

  • Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships, and team building. Following hospital and department policy and procedures and contributing to overall quality services.
  • Works cooperatively with others and organizes daily workflow to optimize output.
  • Completes medication histories for new admissions.
  • Performs discharge duties from admission of patient to final discharge plan. This includes collaborating with inpatient floor staff and outpatient pharmacy staff on delivery and timing of delivery.
  • Performs general duties of a pharmacy technician including but not limited to:
  • Provides service to customers to meet or exceed their needs and expectations.
  • Collects and compiles data, statistics and other information needed for quality assurance programs or reports to provide information to assist with decision-making.
  • Participates in the team approach to quality improvement to support the Pharmacy Department's commitment to the philosophy and principles of Quality and Safety.
  • Maintains updated knowledge and skills and contributes to the education of others.
  • Participates in planning processes by establishing personal goals to support quality improvement efforts and contribute to the achievement of departmental objectives.
  • Provides age/developmentally appropriate care in accordance with Age/Developmental Specific Care Guidelines for the age groups served when participating in patient care

Minimum Qualifications

Education: High School Diploma or Equivalent

Experience: Two (2) years of Pharmacy Technician Experience. Medication Reconciliation Technician or Meds to Beds experience is preferred.

Equivalency:

  • Completion of pharmacy technician program may substitute for one (1) year of the experience requirement. OR
  • Completion of pharmacy technician program plus one (1) year of experience performing medication prior authorizations may substitute for two (2) years of pharmacy technician experience.

Certification(s):

  • State of Colorado Pharmacy Technician (PHAT) Certification
  • Certificate through a nationally recognized certification board or body approved by the National Association of Boards of Pharmacy (NABP) such as the Pharmacy Technician Certification Board (PTCB) or National Healthcare Association (NHA)
  • Medication History Certificate within 12 months of hire
