Position Summary / Department Summary:
Seeking a dynamic and collaborative Patient Navigator to join Boston Children's Hospital Accountable Care Organization (ACO)'s MassHealth Health Related Social Needs (HRSN) Services Program. This role focuses on addressing social determinants of health for children, teens, and young adults, and their families facing nutrition and/or housing insecurity across four BCH Primary Care sites. The Patient Navigator will provide case management support to HRSN-enrolled patients and families, working closely with the HRSN centralized team, hospital-wide interdisciplinary teams, primary care sites, ACO staff, and community partners. The position is hybrid.

Key Responsibilities:


  • Conduct initial needs assessments for patients and families enrolled in HRSN.

  • Develop goals and benchmarks for patient care and interventions.

  • Plan and coordinate appropriate services or interventions.

  • Monitor progress and service delivery for patients and families.

  • Advocate for patients in navigating social systems.

  • Maintain up-to-date documentation and timely data collection in EHR.

  • Provide enrollment support to the HRSN enrollment team as needed.

  • Collaborate with interdisciplinary teams, primary care sites, ACO staff, and community partners to ensure comprehensive care.



Minimum Qualifications

Education:


  • High school diploma or GED required.

  • Bachelor's Degree in Human Services or a related field preferred.


Experience:


  • Minimum of one year of closely-related experience required.

  • Experience working with clinic populations or community housing/nutrition resources preferred.


Licensure / Certifications:


  • No specific licensure or certification required.

  • Fluency in Haitian Creole, Cape Verdean Creole, and/or Spanish strongly preferred.

  • Knowledge of local and state-wide social services resources strongly encouraged.
