A Brief Overview
Primary responsibilities include coordination of patient care, patient/ family education, managing and triaging patient care phone calls within the RN scope of practice. Must be highly organized and able to pay close attention to detail, including distinguishing among priority items. Must have the ability to work independently and manage difficult situations with skill, patience, and diplomacy. The nurse must demonstrate competence in caring for patients with a wide variety of health issues. He/She assists the Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Manager and Ambulatory Nursing Director for nursing practice issues. May be responsible for overseeing care coordination efforts delegated to others within the practice.

Ideal candidate would have experience with data collection using Epic and/or REDCap.

  • Follows the Nursing Process and to deliver safe care: assessment, planning, implementation and evaluation
  • Collects and synthesizes complex assessment data and uses critical thinking skills to best determine most appropriate protocol, disposition, and action
  • Develops a plan of care, as indicated
  • Communicates and collaborates with families/caregivers on plan of care and ensures understanding and compliance
  • Documentation reflects all aspects of telephone encounters
  • Collaborates with the PCP and other healthcare providers as necessary
  • Professionalism/Leadership
  • Role models appearance and behaviors that promote a positive image of Nursing.
  • Role models how to interact with parents/patients and colleagues with respect and dignity.

  • Leads unit improvements and measures outcomes.
  • Assures that staff incorporates professional standards of practice and regulatory requirements into nursing care provided.
  • Follows hospital policies regarding HIPAA and privacy.
  • Research
  • Reads, analyzes and synthesizes nursing and other scholarly articles and research studies; shares knowledge with unit staff in a formal format.
  • With assistance applies research findings to clinical practice.
  • Seeks opportunities to participate in research studies or answer simple research question.
  • Learning
  • Identifies learning needs of self and others and assists in the development of a learning plan.
  • Applies education and training from departmental in-services and external sources to enhance Nursing practice.
  • Assists with developing and utilizes patient education materials relevant to patient population
  • Participates in development/ presentation of staff education programs geared to meet practice and select departmental needs.
  • Demonstrates health care informatics competency in order to manage patient care
  • Information literacy, including the effective use of the intranet and internet to find electronic resources for patient care, nursing practice and evidence-based decision-making;
  • Information management skills, including the effective use of the electronic health record and clinical information systems, the ability to collect clinical data using quality tools and the ability to discuss, evaluate and analyze outcome data presented in charts and tables.
  • Resource Management
  • Participates in promoting quality, efficient and cost effective care.
  • Monitors own usage of supplies and equipment/identify opportunities for improvement and makes recommendations.
  • Manages own time to accomplish direct/indirect care in a cost efficient manner.
  • General Responsibilities
  • Completes letters of medical necessity, referrals, and prior authorizations as needed.
  • Facilitates prescription refills per office protocol
  • Assists with acquisition of DME, as indicated
  • Conducts telephone triage and refer patients to appropriate services.
  • Assists patients and families with access to health care services as needed, including collaboration with payers.
  • Coordinates plans for follow up visits with care providers and for laboratory and diagnostic studies.
  • Initiates care coordination based on identified needs per discussion with patient/family
  • Advocates for patients and families and promotes communication and collaboration to keep all care team care members informed of patients' care needs and to ensure that patients access and benefit maximally from available health care resources.
  • Ensures adequate and timely response to all requests for information and service by customers that present in person, by telephone, or other message.
  • Answers telephones, routes and screens callers; takes messages and provides routine information to callers promptly and courteously.
  • Interprets basic laboratory tests and in conjunction with primary care provider, establishes and communicates a plan of care with families.

Education Qualifications

  • Technical Diploma Nursing - Required
  • Bachelor's Degree Nursing - Preferred

Experience Qualifications

  • At least three (3) years nursing experience - Required
  • Experience in pediatric primary care, care coordination, telephone triage using Barton Schmitt protocols, Epic - Preferred

  • Ability to locate and review specific patient data in various clinical information systems.
  • Effectively uses clinical decision support tools.
  • Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.
  • Ability to collect clinical data using quality improvement tools.
