Position Title: Advanced Practice Provider (APP), Geriatrics The Section of Geriatrics at Boston University Chobanian & Avedisian School of Medicine and Boston Medical Center (BMC) is seeking an APP to join our faculty. Our academic medical practice is committed to excellence in clinical care, teaching, and research of older adults. We provide the utmost quality clinical care in a variety of settings, including an ambulatory practice, homecare, and nursing homes. We are seeking a dynamic APP with experience in adult, geriatric, or family medicine for our Nursing Home Program. In collaboration with an attending physician, the APP will provide primary care and clinically manage a panel of long-term care residents living in a skilled nursing facility and manage medically complex and active short-term sub-acute patients. The APP will perform admissions, follow up and discharge visits for subacute rehabilitation patients; conduct periodic and regulatory visits for nursing home residents; order lab work and diagnostics as deemed appropriate; review and reconcile medications and sign orders. The APP will document in both the nursing home record and BMC EMR (EPIC). Visits are approximately 30 to 60 minutes, depending on the acuity of the patient. This role offers flexibility, autonomy, and APPs make their own daily schedule. You will work collaboratively with peer APPs/MDs. Job Requirements NP or PA certification is required in Adult, Geriatric, or Family Medicine. A minimum of 3 years of practice experience. Candidate must possess strong clinical assessment and communication skills; experience with managed care and HCC Risk adjustment methodology is a plus. Candidate will need to be credentialed at Boston Medical Center and will need a current MA professional license, Federal DEA, and Massachusetts Controlled Substance Registration.
