  • Hillcrest Hospital South is a full-service 180-bed hospital providing a wide range of inpatient and outpatient services using state-of-the-art technology in a friendly community setting. Consistent performance has helped the hospital be nationally recognized for patient safety.

Responsibilities

  • Responsible for providing social work services to patients and families to help them manage and cope with the psychosocial implications and impact of illness.
  • The social worker assists with communication, counseling, navigation of systems of care, providing resources, and providing discharge-planning services to patient/family.
  • Addresses the needs of both patient and family through counseling, education and community resource linkage, serving as an advocate for patients/families. Responsible for concurrent knowledge of community resources, and identifies discharge and transitional needs.
  • Responsible for the coordination of discharge planning activities in conjunction with the RN Case Manager.
  • Works collaboratively with interdisciplinary staff internal and external to the organization.
  • Participates in quality improvement and evaluation process.
  • Provides support for crisis intervention, including adjustment to hospitalization, disability, chronic illness, mental illness, and abuse.

Qualifications

Job Requirements

  • Master's of Social Work degree

Preferred Job Requirements

  • Previous hospital experience
  • Experience in Acute Care
