JOB DESCRIPTION:

SUMMARY OF POSITION

Provides support and stabilization services to clients on crisis residential unit. Provides peer and crisis support services, information, education and support for clients. Promotes wellness, recovery and skill building to sustain better health through prevention training for managing stress and problems, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving, and sustaining recovery. Assists clients with personal recovery goals, helps clients develop natural support systems, and coaches clients to advocate for themselves within their healthcare systems. Implements evidence-informed wellness interventions.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides group or individual training/interventions, in crisis residential unit, focusing on improvement of the client's functioning and health-related issues (e.g., symptom/medication management, life skills, healthy eating, weight management, smoking cessation, recognizing the dangers of alcohol and other drugs, community reintegration, etc.).
  • Assesss level of risk for clients in crisis and assist clients in developing crisis safety plans. Provide wellness consultation. Work in consultation/collaboration with clients, family members and other providers to implement plan.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals. Assist clients with finding solutions utilizing motivational interviewing and finding motivation to complete their individualized recovery plan to achieve wellness goals. Assist clients in overcoming any barriers to goals, including access to care. Assist with navigating health care systems.
  • Links clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
  • Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.
  • Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned.
  • Assist clients with food preparation, and Activities of Daily Living (ADL) and Instumental Activities of Daily Living (IADL) for example: Sweeping, mopping, fridge clean out, etc.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of community/geographical awareness and of community resources.
  • Knowledge of and ability to instruct assigned consumers in basic independent living skills.
  • Skill in crisis intervention.
  • Skill in providing advocacy services.

QUALIFICATIONS

Education Level

Candidate must be 21 years of age per funding requirements.

Must meet Mental Health Professional Status based on Section 140.

Bachelor's Degree in human services related field preferred.

Education/experience equivilancy may be considered upon approval.

Years of Experience

Two years relevant experience in human services; as well as experience participating in their own personal recovery process in order to care for self or family member.

$18.20--$25.50
