POSITION SUMMARY: The Community Health Worker (CHW) supports patients by connecting them to community resources, providing education, coordinating care, and assisting with follow-up and engagement. Working under the Social Work Manager, the CHW collaborates with an interdisciplinary team to improve health outcomes and participates in community outreach activities. Reliable transportation is required for community-based travel. QUALIFICATIONS:
High school diploma or equivalent required; knowledge of East Baltimore community resources preferred.
Strong interpersonal, communication, organizational, and problem-solving skills.
Ability to work independently, use sound judgment, and engage patients effectively.
Basic computer and data entry skills; familiarity with medical or social service settings preferred.
Valid Maryland driver's license and reliable transportation required if using a personal vehicle.
Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
