Registered Nurse Lead Washington, DC Pay From: $45 per hour MUST: Experienced Registered Nurse with strong Lead experience 2+ years of professional experience as a Registered Nurse 2+ years of Leadership and/or management experience Experience working with un-housed populations, substance use disorder and un(der)treated mental illness Recent acute care experience in medical Emergency, Urgent care, or other inpatient units Experience in/passion for public health and community nursing Ability to perform well in high-pressure or stressful situations Excellent communication and interpersonal skills Excellent computer skills and proficiency with the Microsoft Office product suite Independent decision maker and able to lead and manage people in a motivational and constructive way, individually and in a group setting. Experience with the following EMRs is a plus: eCW, Epic Current DC Registered Nurse license Current BLS/CPR certification Graduate of an accredited school of nursing with a Bachelor's in Nursing (BSN) DUTIES: As the lead on-site clinical nursing role, provides supervision, mentorship and oversight to other nursing roles, documenting and reporting performance concerns to the Nurse Manager. Assesses, plans, implements, and evaluates the care of the patient Works with the provider and staff nurse in developing and implementing treatment plans for patients. Assures that nursing documentation, including both electronic documentation in the Electronic Medical Record and on-paper documentation of Medication Administration and nursing interventions, are completed per policy and standards. Reviews the documentation of the other Respite nursing staff (RNs, LPNs, CNAs) routinely to assure it is appropriate and timely. Performs and/or delegates and supervises the performance of primary nursing procedures to other clinical staff as appropriate given their license and scope of practice, including but not limited to daily vital signs, medication administration, wound care and other nursing interventions Ensures that medical care and follow up are planned and documented and that all requested for referral services are completed for each patient. Develops systems to manage care coordination for medical respite patients: management of referrals, tracking of outside specialists, transportation to/from outside appointments. Researches and makes recommendations to program leadership about best practices for accessing, scheduling and coordinating transportation for unhoused patients to/from medical appointments in the District. Supports patients to advocate for themselves in their interactions with these other community providers and stays abreast of efforts to house or transition patients to other long-term placements. Designs, implements and monitors systems necessary for the safe, efficient functioning of the program. With support from the Nurse Manager, manages supply chain and storage for medical supplies and emergency equipment. Maintains a physical environment of care and a team culture that measures and promotes safety and quality, from hand hygiene to daily quality controls. In partnership with the Nurse Manager and Provider, identifies, measures and critically evaluates program process and outcomes measures periodically. Conducts chart audits for all nursing roles according to the number and rhythm defined by the Nurse Manager. Utilizes strength-based patient-centered motivational interviewing techniques to build rapport and help patients improve their health. Participates in the development, maintenance, and adjustment of individualized care plans for high-risk patients that address both medical and social barriers to accessing care. Educate client on medication administration and management, overall health management and condition monitoring. Identify and implement strategies to support independence in health management Develop, plan and facilitate group education focused on a range of health topics relevant to the patient population.
