Summary Transitions of Care Pharmacist IFull-Time-First Shift BHCOR-Pharmacy I Corbin, KY The Pharmacy provides pharmaceutical services to patients across the continuum of care. Services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation and a meds to beds program for inpatients. Baptist Health Pharmacy services are committed to ensuring the optimal, safe, and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value-based environment. Transitions of care pharmacists are responsible for target interdisciplinary programs and clinical services and ensures pharmaceutical care programs are appropriately integrated throughout BH system. In these clinical roles, transitions of care pharmacists participate in all necessary aspects of the medication-use system while supporting comprehensive and individualized pharmaceutical care in their assigned areas. They also serve as clinical resources and liaisons to other departments, hospital personnel, or external groups. In their role, they may conduct practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Medication Use & Formulary Management
Requirements: * BS degree and one year Experience required. Kentucky and Indiana Pharmacist License Registration with Kentucky Board of Pharmacy (KY hospitals), within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire. Benefits: Full-Time 32 weekly hours (0.8 FTE) - 40 weekly hours (1.0 FTE)
