Job Description

The Senior Advisor, Clinical Care will support the Transitions of Autism Care (TRAC) Project, a multi-year project focused on improving healthcare transitions for autistic adolescents and young adults as they transition from pediatric to adult-serving systems. This role is designed for individuals with lived experience and will focus on providing peer support as part of a transition to adulthood consultation model, program development, community outreach, and evaluation. The role may include planning, organizing, and evaluating the data for care innovation in clinical settings and will support strategic goals for growth in caring for individuals with intellectual and developmental disabilities.

Job Responsibility



  • Support individuals and families in navigating health and social systems including healthcare, disability services, education, employment, and housing
  • Participate in interdisciplinary team meetings to coordinate care and provide insights from a lived experience perspective
  • Build trusting relationships with individuals referred to the TRAC consult service and provide continuity of peer support throughout the transition process
  • Act as a liaison between healthcare providers and community-based organizations to ensure alignment of care and referrals
  • Promote awareness and utilization of the TRAC consult service across departments and among community stakeholders
  • Assist in developing future peer and family navigation training programs for individuals with autism and their caregivers, in alignment with TRAC project's long-term workforce development goals
  • Contribute to program evaluation, documentation, and quality improvement effort
  • Develop and articulate a short-term strategic vision for areas of responsibility
  • Develop goals and initiatives to promote service excellence for patients and clinicians in the catchment area
  • Identify opportunities for operational improvements and/or the expansion of services; recommend improvements and strategies; implement approved enhancement
  • Develops and leverages strategic business relationships and networking opportunities with entities including but not limited to, physicians, medical/group practices, business leaders, and community-based organizations, to generate new referral source relationships.

Job Qualification

Bachelor's Degree in Nursing, required.

10+ years of relevant experience, required.

Experience working with individuals with intellectual or developmental disabilities, preferred

Clinical experience and Project Development experience, preferred

Ability to work part-time hours, 3 days per week, preferred
