What We Do We typically care for up to five patients a day. We start and end our day with virtual team rounds so we can stay connected and aligned. Throughout the day, we communicate regularly to support patient needs, coordinate care, and ensure continuity. ________________________________________ Key Responsibilities Provide high-impact, patient-centered care by completing thorough nursing assessments and creating individualized care plans. Deliver advanced, skilled nursing interventions-including treatments, education, and inpatient-level clinical support-in the patient's home. Monitor patient status using remote physiologic monitoring and respond promptly to changes in condition. Coordinate comprehensive care, including diagnostics, medication reconciliation, durable medical equipment (DME), and interdisciplinary services. Document care efficiently and accurately using Point-of-Care electronic records. Provide patient and caregiver teaching, support safe transitions, and assist with discharge planning. *Manage your daily schedule with autonomy, aligned with patient needs and program priorities. ________________________________________ Graduate of an approved, accredited school of professional nursing required; BSN preferred Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required Current CPR certification required Minimum 1 year of recent acute care, home care or emergency nursing experience required Strong clinical assessment and critical-thinking skills Ability to work independently and adapt to changing patient needs Proficiency with electronic documentation and mobile technology Current driver's license, a good driving record, and reliable transportation required Mass General Brigham Home Care, Inc. is an Equal Opportunity Employer.
