Job Title: Patient Navigator Specialist Location(s): Oakland, CA Position Summary The Aftercare Coordinator serves as a dedicated Patient Navigator within a trauma program, leading the implementation of a hub-and-spoke model to support children and families following traumatic brain injury (TBI). The role bridges the gap between inpatient care and long-term recovery by guiding patients through post-acute services such as neuro-recovery follow-up, school reintegration, rehabilitation, and community-based support. Acting as a central point of contact, the Coordinator ensures continuity of care, empowers families with education and advocacy, and contributes to a trauma survivorship framework with a specialized focus on TBI.
Key Responsibilities Patient Navigation & Family Support
- Serve as the primary liaison for patients and families throughout the TBI recovery process.
- Provide emotional support, education, and tailored resource navigation.
- Facilitate transitions between inpatient, outpatient, and community-based services.
- Support school reintegration and long-term adaptation following TBI.
Program Development & Coordination
- Design and implement program components in collaboration with clinical and operational leadership.
- Build and maintain partnerships with internal departments and external agencies.
- Coordinate multidisciplinary care to ensure integrated and efficient service delivery.
- Contribute to program strategy with emphasis on survivorship and recovery outcomes.
Project Management & Administrative Support
- Develop and manage project plans, charters, timelines, budgets, and status reports.
- Track deliverables, risks, resource requirements, and interdependencies.
- Prepare presentations, facilitate meetings, and document decisions and outcomes.
- Ensure confidentiality and data accuracy in all project documentation.
Education & Training
- Assist in the development of educational curricula and materials for staff and families.
- Deliver training to internal stakeholders and community partners.
- Serve as a content resource regarding TBI recovery and survivorship navigation.
Quality Improvement & Community Engagement
- Participate in trauma-related quality improvement initiatives.
- Support evening and weekend community events as needed.
- Contribute to data collection, evaluation, and outcome reporting.
Additional Requirements
- Flexibility for occasional evening or weekend assignments.
- Active involvement in program evaluation and reporting.
- Other duties as assigned to support trauma program effectiveness and patient-centered care.
