Organization: Lutheran Social Services of WI and Upper MI Location: Outagamie County, WI Schedule: Full-time, Monday-Friday, first shift (8am-4pm or 9am-5pm) Flexibility: Required to meet family needs Benefits: Full-time benefit eligible! Lutheran Social Services is seeking a Care Coordinator to support families through the Children's Long-Term Support (CLTS) Waiver Program-a federally funded Medicaid program serving children with developmental, physical, or severe emotional disabilities. As a CLTS Care Coordinator, you'll be the main point of contact for families, helping them access services that keep children safe and thriving in their home and community.
