SUMMARY: Brown University Health employees are expected to successfully role model the organization's values of Compassion, Accountability, Respect, and Excellence as these values guide our everyday actions with patients, customers and one another. * Maintain knowledge of the Healthy Transition's Coordinated Specialty Care evidence-based treatment model. * Conduct client prescreening to determine eligibility and initial assessments to identify primary psychosis and symptom profile related to diagnosis * Complete intake and onboarding of new clients, obtain history, risk factor information, and involve families when appropriate to collect information * Provide psychoeducation to client and families * Support clients as they work towards their employment and educational goals * Use creative approaches and Motivational Interviewing to engage clients who may be ambivalent about participating in treatment. * Provide clinical services as identified in the treatment plan, in the home, at a Center facility, and in the community. * Assist clients to clarify goals, learn new skills to support behavior change, improve social and vocational skills and increase coping strategies * Remain current on available community resources relevant to clinical assignment and accesses them as needed * Maintain ongoing and collaborative relationship with client, team members, and community providers and partners * Develop treatment plans and initiate clinical interventions utilizing individual and group treatment and analyzes course and results of treatment * Participate in case consultation, collaborative care meetings, and integrated treatment planning, providing consultation regarding client's background, psychosocial functioning, and needs. * Present and discuss program at community locations, job sites, hospitals, and with general practitioners for the purpose of outreach and recruitment * Attend coordinated care meetings and report issues to Team Leader. * Attend agency and Healthy Transitions staff meetings as scheduled. * Attend weekly supervision as scheduled. * Maintain detailed, accurate, and timely documentation in medical record. * Enhance and maintain professional expertise through educational activities and trainings. * Master's degree in social work, mental health counseling, or marriage and family therapy and on an independent licensing track with the RI Department of Health * At least 1-year of experience working with targeted population in a mental health setting. NECESSARY SPECIAL REQUIREMENTS: *Analytical skills necessary to evaluate patients' mental health needs and to formulate and implement a treatment plan *Knowledge of adolescent development and family systems issues. *Advocacy and teaching skills. *Valid driver's license and use of car during normal working hours or other means of transportation in order to perform the major functions of the job. *Automobile liability insurance coverage for vehicle being used in accordance with Center requirements. *CPR training obtained within 90 days of employment and maintained annually. *Ability to work collaboratively as a team to coordinate care for youth and young adults with intensive needs.
