Specialty Pharmacist - Ambulatory

Responsible for providing medication therapy management services to assure that drug therapy is safe, effective, efficient and cost effective. Assists patients, providers and partners with chronic disease state management and education as part of an interdisciplinary team. Completes patient assessments and follow-up necessary to comply with specialty pharmacy accreditation standards. Collects and analyzes data for process improvement projects. Responsible for assisting or leading projects involving specialty pharmacy which may include sharing information and improving processes with retail pharmacy partners. Will be required to take after hours call on a rotation basis. Performs other duties as assigned or as necessity dictates. EDUCATION AND EXPERIENCE Minimum - Graduate of an accredited Pharmacy program Preferred - Completion of an ASHP Accredited Residency training program CERTIFICATION/LICENSURE Pharmacist license (or eligible for licensure) in the State of Illinois and/or Wisconsin based on practice setting Pharmacist license in both Wisconsin and Illinois may be required Approved CPR or BLS certification required within 90 days ESSENTIAL DUTIES AND RESPONSIBILITIES
