Population Health & Equity Specialist (PHE)
needed for a temp hybrid contract
opportunity with Yoh's non-profit Medi-Cal managed care health plan client located in Alameda, CA.
This will be no more than 6 months (or 1,000 hours, whichever comes first).

Support the company's mission to improve community health by developing, implementing, and evaluating programs that advance population health and reduce health disparities. Core focus areas include: program development & implementation, program evaluation & monitoring, and health equity advancement.

Top Skills You Should Possess:

  • Minimum of two years of relevant project management and health promotion experience
  • Proficiency in Windows including Outlook, Excel, Word, and PowerPoint
  • High degree of independence, flexibility, initiative, and commitment
  • Solid critical thinking and problem-solving skills
  • Ability to work in a fast-paced, member-service oriented environment



What You'll Be Doing:

  • Assist in the planning, execution, and evaluation of health programs for members
  • Address health inequalities through health promotion activities that meet the cultural and linguistic needs of the diverse membership
  • Compile and analyze qualitative and quantitative data and prepare concise and clear narrative reports
  • Create materials, handouts, newsletter articles, presentations and website content for staff, member, and provider audiences
  • Develop and maintain working relationships with community organizations, vendors and provider partners and participate in community coalition
  • Plan and facilitate meetings and training activities with members, providers, community partners and staff
  • Plan and conduct focus groups and surveys to ensure member and provider input into program development



What You Need to Bring to the Table:

  • Must be able to work Monday - Friday 8:00am to 5:00pm with 1-2 days a week being remote (depending on business needs)
  • Master's degree from a health or related field (preferably with specialization in public or community health education) OR a Bachelor's degree accompanied with three years of work experience in population health, community health education, or health equity
  • Preferred: Certified Health Education Specialist (CHES) Credential
  • Ability to utilize sound judgment and promptly report potential risks
