Works collaboratively with patients, families, the interdisciplinary team and external providers to ensure safe, timely, cost-effective care. Develops and oversees individualized care plans, serves as a problemsolver and resource, and drives continuous improvement in patient outcomes. Strong internal communication skills are required to gain cooperation and implement change. KEY RESPONSIBILITIES

Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
