FM Inpatient Navigator Family Community Medicine Full Time 88523BR Job Summary The Family Medicine Inpatient Navigator will support the clinical and logistical needs of patients admitted to the Family Medicine Inpatient and Parent Baby Services as they transition from the hospital to home and other community settings. Despite a long history of caring for vulnerable and marginalized patients with complex chronic illness, as well as extensive efforts by our physicians, pharmacists, nurses, social work and nursing partners, we continue to have difficulty safely bridging patients to the community for continued care and mitigating unnecessary readmissions. Our aggregate 30-day readmission rate has at times been as high as 20-25%, a level well above Centers for Medicare & Medicaid Services (CMS) targets. This position helps to monitor this rate and improve it by focusing resources and support on collaborative planning and communication between patient, inpatient and outpatient clinical teams and community-based organizations. The FM Inpatient Navigator will serve patients and their designees to ensure safe discharge and re-integration into the community during and after discharge from the hospital. The navigator will be an integral part of the care team, attending daily multidisciplinary rounds and working closely with physicians, nurse practitioners, nurses, pharmacists and social workers. Specific tasks for the position are outlined below. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $29.99 - $48.23 (Hourly Rate). The Behavioral Diabetes Research Group (BDRG) is a multi-faceted program engaged in practice based trainings, research, and electronic health-related tools, in the Department of Family & Community Medicine. The mission of the BDRG is to gather new knowledge about the behavioral, social, emotional, cultural, and familial aspects of diabetes and its management over time, to develop and implement programs in primary care clinics to help patients with diabetes and their families manage diabetes effectively, and to help providers better organize and configure care to achieve these goals.
